You must book your stay between November 15 and December 6 for a 3 night or longer stay completed by January 15, 2013 at a hotel in one of the participating regions.
The $50 restaurant gift card is valid at these dining brands:

There is no promotion registration required. After your qualifying hotel stay, you must fill out the Choice Hotels rebate form and return it with a copy of your hotel receipt postmarked by January 31, 2013.
The restaurant gift card is a rebate on top of the normal points earning as a Choice Privileges member for your Choice Hotels stay.
This offer may be earned one time per stay. The terms look like you can earn this offer multiple times on separate 3-night stays during the promotion period.
